Step-by-Step Guide
1. Access Profile Screen: From the app's bottom navigation, tap on the 'Profile' icon.
2. Navigate to Connected Portals: Once on the profile screen, find and select 'Connected Patient Portals'.
3. Initiate Disconnect: Locate the portal you wish to disconnect and tap 'Remove'.
4. Choose Data Retention Option: Upon tapping 'Remove', you'll be presented with two choices:
- Remove Portal and Keep Data: This will disconnect the portal but retain all data previously fetched from it.
- Remove Portal and Delete Data: This will disconnect the portal and aim to delete associated data.
Important Note: Even if you opt to 'Remove Portal and Delete Data', we might still retain some of the data. If you were previously associated with a care provider, data might be stored by SigmaMD or its service providers on their behalf.
