- Launch the App to Create an Account
- Once downloaded and installed, launch the SigmaMD app and tap on the 'Create Account' button.
- Enter Your Email
- Fill in your email address.
- You will receive a verification code; enter this code to verify your email address.
- Set Up a Password
- Create a secure password. It must be a minimum of 8 characters long and contain at least one letter, one number, and one special character.
- Enter Your Preferred Name
- Fill in your preferred name.
- Set Up 2-Step Verification
- Enter your phone number.
- Verify the number using the code sent to your phone.
- Review and Accept Policies
- Check the box to accept the SigmaMD Privacy Policy and Authorization for Health Information.
- Scroll through the Terms of Service and tap on 'I Agree'
Automatic Association with Your Care Provider’s Practice
If your account is created using the same phone number that your care provider entered in the SigmaMD Clinician when adding you as a patient, you will be automatically associated with your care provider’s practice. This allows for smooth communication and management of healthcare details between you and your care provider.
